Botulinum toxin: what it is and why people call it Botox

Botox is a brand. Botulinum toxin is the substance.

Confusing the two is the same habit as calling every tissue a Kleenex or every adhesive bandage a Band-Aid: the first brand to arrive became the generic name. Botox® is one commercial brand of botulinum toxin type A, and several others are registered in Brazil, with slightly different formulations and behaviour.

In the treatment room this matters little to the person receiving it — they all do essentially the same thing. But it is worth knowing that when someone says "I get Botox", they are saying "I have botulinum toxin injected", and the product used could be any of the registered ones.

What botulinum toxin is

It is a protein produced by the bacterium Clostridium botulinum. In the quantity and route of exposure typical of food poisoning, that same protein causes botulism — a severe paralysis.

This tends to alarm people, and the explanation is the usual one in pharmacology: the dose separates the poison from the medicine. The amount used in facial aesthetic treatment is thousands of times smaller than what would be needed for a systemic effect, and it is purified, standardised in units and injected locally into the target muscle.

The same reasoning applies to plenty of everyday medications that are toxic at a high dose.

How botulinum toxin acts on muscle

For a muscle to contract, the nerve has to release a neurotransmitter — acetylcholine — at the junction between nerve and muscle. The toxin blocks that release.

Without the signal, the muscle contracts with less force. The skin that was being folded by that movement stops folding, and the expression line softens.

In facial aesthetics

It treats dynamic wrinkles — the ones that appear with movement. Forehead, glabella, crow's feet, gummy smile, and the contour of the lower third via the masseter.

It does not treat deep static wrinkles, the ones already etched with the face at rest. That is a limitation of mechanism, not of dose: the toxin acts on muscle, and a set line belongs to the skin.

In therapeutic indications

Medical use of the toxin predates the aesthetic one and is broader than most people imagine: chronic migraine, hyperhidrosis, muscle spasms, dystonias, strabismus, overactive bladder, among others.

At the interface with dentistry, the most relevant functional use is bruxism — injecting the masseter reduces clenching force, with an effect on muscle pain and tooth wear. The slimming of the facial contour, in that case, is a consequence of the functional treatment.

What the risks of botulinum toxin are

The most common events are local and short-lived: a small bruise at the injection point, tenderness, a mild headache in the first few days.

The unwanted effects that bother people most come from diffusion into neighbouring muscles or from poorly chosen points:

All are temporary. All depend more on technique and anatomical knowledge than on the product. And none is reversible — unlike filler, toxin has no antidote, and the treatment is time.

Contraindications

Neuromuscular diseases such as myasthenia gravis, Eaton-Lambert syndrome and amyotrophic lateral sclerosis; allergy to components of the formulation; active infection at the site; pregnancy and breastfeeding.

Does the toxin stay in the body?

It does not accumulate. The protein is broken down by the body, and what marks the end of the effect is the rebuilding of the nerve endings — not the substance "leaving".

What can accumulate, in a minority of people, are neutralising antibodies, when sessions are very frequent or involve repeated high doses. That is the reason for the minimum three-month interval between sessions.

Botulinum toxin and the liver

There is no mechanism by which toxin used in facial aesthetics would cause liver damage. The dose is minimal, the action is local at the neuromuscular junction, and no meaningful hepatic metabolism is involved.

The question circulates widely and has no basis — but liver conditions, like any health condition, belong in the medical history.

What the choice of product changes

Every toxin registered with Anvisa, the Brazilian health regulator, for aesthetic use is botulinum toxin type A. The differences between brands lie in the formulation — the presence or absence of accessory proteins, units of measurement that are not interchangeable between products, and small variations in onset and duration of effect.

For the person receiving it, this is secondary. The choice of who injects weighs incomparably more than the choice of brand, because dose, points and reading of the musculature define the result.

What is worth checking is the product's registration and the integrity of the vial — not the brand.
