Microfocused ultrasound delivers heat at depth and stimulates collagen. Results take months, it hurts during, and it does not replace surgery. What to expect.
Microfocused ultrasound is the non-invasive technology with the best evidence for a lifting effect on the face. It is also the most poorly sold, because it tends to be advertised as a "non-surgical facelift" — and that is not what it is.
It is worth understanding what it actually does, because the gap between the promise and the mechanism is wide.
How microfocused ultrasound works
The device concentrates ultrasound energy at microscopic points, at set depths. At those points, the temperature rises enough to produce a controlled thermal injury — small zones of coagulation, without breaking the surface of the skin.
The two effects
The body responds to these micro-injuries in two ways:
- Immediate contraction of existing collagen fibers, from the heat.
- Neocollagenesis — new collagen produced over the following months, as part of the repair process.
The depth that sets it apart
What distinguishes this technology is depth. It reaches layers that other technologies cannot reach non-invasively, including the plane of the superficial musculoaponeurotic system — the same layer a facelift repositions.
Reaching that layer, though, is not the same as repositioning it.
What to expect from the result
Here is the part that needs to be said honestly.
Firmness, not repositioning
The result is firmness and definition, not repositioning. The skin becomes firmer, the jawline gains some definition, and the treated area looks better supported.
It appears slowly. There is a subtle immediate effect from the thermal contraction, but the real result depends on collagen production and takes two to three months to show, continuing to develop after that.
It is modest compared with surgery. Anyone expecting the result of a facelift will be disappointed — not because the device is poor, but because these are different kinds of intervention.
A useful way to calibrate expectations: microfocused ultrasound is excellent for postponing the conversation about surgery in someone with mild to moderate laxity. It is not an alternative to surgery in someone already at the advanced end.
Does microfocused ultrasound hurt?
Yes, and more than most non-invasive aesthetic procedures.
The sensation is deep pricks of heat, synchronized with the pulses of the device. It is more intense over bony areas and along the jaw.
What is used to manage it: pre-treatment analgesia, parameter adjustment, and breaks. The session usually runs thirty to ninety minutes, depending on the areas.
This is worth knowing beforehand. It is a procedure with no downtime and real discomfort during — the reverse of the combination most people expect.
How the face looks afterward
No meaningful downtime. There may be redness for a few hours, mild swelling, and tenderness to the touch for a few days.
One possible complaint is muscle soreness in the treated area for up to two weeks, similar to the feeling of a muscle that has been worked.
Normal routine resumes the same day, which makes it a viable option for anyone who cannot take time off.
How many sessions, and how often
Unlike other technologies, it is usually done as a single session, repeated annually or according to the response.
That changes the cost comparison: one microfocused ultrasound session costs more than one radiofrequency session, but radiofrequency requires a series of them. The number that matters is the cost of the full protocol.

All three stimulate collagen by different routes, and they are not interchangeable.

In practice they are often combined: the device works on support, and the injectable restores the structural volume that has been lost. Neither does the other's job.
When we advise against it
- In advanced laxity. It does not deliver, and proposing it in that scenario is selling an expectation that will not be met. We refer out.
- When the expectation is a facelift. The technology does not reposition tissue. If that is what the person wants, it is a different conversation.
- When the complaint is volume loss. Stimulating collagen in a structure with no support delivers little. Restoring structure comes first.
- With no time for the result to appear. An event in four weeks does not fit a procedure that takes two to three months.
- Over recent filler in the area, because of the effect of heat on the product. The interval has to be respected in both directions.
- On skin with active infection, an open lesion, or an inflammatory process in the area.
