Skin age: what the test measures and what it leaves out

Apps give your skin an age. What that number adds up, the six signs you can assess in the mirror, and which one decides what to treat first.

Skin age tests got popular because they deliver something irresistible: a number. You take a selfie, the algorithm analyzes it, and the screen tells you your skin is 34, or 41, or 29.

The number is fun, but it barely helps. It rolls things that have nothing to do with each other into a single result: sun spots, expression lines, pores, sagging, volume loss. Two people with the same result may need opposite approaches. That is why, when someone arrives at a consultation with a screenshot from the app, the question we ask is a different one: which sign is pushing that number up?

That is what this article is about: what skin age actually measures, how to assess yours without an app, and what really changes the result.

Chronological age vs skin age

Chronological age is the one on your ID. Skin age is how old the skin looks and how it behaves: firmness, texture, evenness of color, how quickly it recovers.

The two drift apart because skin ages along two paths at once:

When a test says your skin "is older than you are", it is almost always measuring the second path. That is good news, because extrinsic aging is precisely the part you can prevent and, to a large extent, treat.

How to test your facial skin age

Apps estimate age by comparing your photo against image banks of faces of known age. It works as a curiosity, with three practical problems: lighting, camera and makeup all change the result. The same person, with the same skin, can get different numbers in two different settings on the same day.

If you want to use the app, use it as a comparison with yourself: same light, same time of day, no makeup, every so often. As a diagnosis, it is useless.

The most informative test happens in the mirror, with natural light coming from the front, looking at six signs separately:

Note which one stands out most. That one, not the average, decides where to start.

What ages facial skin?

Sun, first and foremost. Ultraviolet radiation breaks down collagen, disrupts pigment production and builds up damage over the years, including on cloudy days and inside the car. The difference between the skin on someone's face and the skin on the inside of their arm shows clearly what the sun does, because both have the same age and the same genetics.

Smoking. It reduces oxygen supply to the skin and speeds up collagen breakdown. It shows in the lines around the mouth and in a grayish tone.

Poor sleep and chronic stress. They impair skin recovery and the barrier. On their own they weigh less than the sun, but they add up.

Too much sugar. It drives a process called glycation, in which collagen fibers stiffen and lose function.

Loss of structure. Part of what reads as aged skin sits beneath it: fat that shrinks and shifts, bone that remodels, support that gives way. The skin can be in great shape and the face still look tired, because what changed is the structure holding it up.

This last point is the one selfie tests confuse most. They read the shadow of volume loss as "old skin", and no cream fixes that.

Are large pores related to age?

Yes, but the reason changes with age, and that changes the treatment.

In young skin, visible pores are usually about oil: an active gland, a filled pore, a visible opening. In mature skin, the pore enlarges for a different reason: the collagen around it loses firmness, the skin gives way, and the pore stops being round and becomes elongated, teardrop-shaped, mainly on the cheeks.

That is why someone who had pores on their nose at 20 starts noticing pores on their cheeks at 40. The cheek pore is, in large part, a sign of firmness. Treating it as if it were oiliness, with astringents and drying products, only irritates the skin. Here the target is the support around it. At what age does the face start to change, and when does it change most?

There is no exact age, and be wary of anyone who names one. The onset depends on genetics, skin type and, above all, how much sun the skin has had. What can be said is the order in which things usually appear:

The phase in which the face changes fastest, for many women, is the transition to menopause: the hormonal drop accelerates collagen loss and the skin thins at a surprising pace. For men, the change tends to be more gradual.

What age is considered mature skin?

"Mature skin" is a cosmetics category with no clinical definition. In practice, the term is used for skin that has moved past the surface phase and into the firmness phase. It makes more sense to classify skin by the sign it shows than by birth year.

Which skin type ages more slowly?

Oilier and more pigmented skin tends to show wrinkles later: it is thicker, and melanin offers some natural protection from the sun. On the other hand, it usually has more trouble with spots and uneven tone.

Fair, thin skin shows lines earlier and suffers more from the sun. Dry skin shows texture and fine lines before the others.

No type is "better". Each one ages with a different face, and that guides where to pay attention first.

Does skin age say anything about health and longevity?

The "biological age" tests circulating online mix two things. There are tests that try to estimate how the body is aging through blood or DNA markers, and there are photo analyses that estimate skin age. They are different measures.

Skin mainly reflects accumulated exposure, above all to the sun. A person can have sun-aged skin and be in excellent health, and the reverse happens too. Use the mirror to take care of your skin. For your health, the route is testing and clinical follow-up.

What actually changes skin age

Each sign responds to something different. That is why the assessment matters more than the number.

Prevention, for everyone. Daily sunscreen is what most influences skin age over a lifetime, and it is also the thing least used properly: too little, never reapplied, forgetting the neck and ears. A well-built routine comes next, and it is explained in the article on facial skincare.

Spots. They respond to peels and laser, with strict sun protection. Without sunscreen, they come back.

Texture and pores. Peels, laser and skin boosters improve the surface and deep hydration of the skin.

Firmness. The target is collagen. A biostimulator does this by injection. Ultherapy, the microfocused ultrasound we use here, does it with energy: we use all of the device's transducers, and it is the transducer that defines the action. The more superficial ones stimulate collagen, and that is the application that goes into a firmness plan.

Expression lines. Botulinum toxin acts on the contraction that etches the line. Once the line is static, it usually works in combination with skin treatment.

Volume and contour. Filler, when indicated, restores the support the skin cannot recover on its own.

All injectable products used in a procedure must be registered with Anvisa, the Brazilian health regulator, and this can be checked publicly.

How we assess here

The app's number does not go into the plan. What goes in is an analysis of the whole face before treating the complaint. Someone who arrives saying their skin looks "old" may be seeing spots, sagging or volume loss, and each calls for a different path. The complaint shows the symptom. The plan starts from the structure.

The criterion is different too. A good result preserves movement, expression and identity. We do not measure it by "how many years younger" the face looks. Treated skin that looks like real skin is worth more than a face that has lost its own character.

How do you calculate skin age?

What is the difference between biological age and chronological age?

Chronological age is the one on your ID. Biological age tries to estimate how the body is aging through lab tests, and skin age estimates appearance. They are three different measures, and the selfie test only measures the last one.

Are skin age apps reliable?

They work as a comparison with yourself, under the same conditions, over time. As a diagnosis, no. They blend signs that call for different treatments into a single number.

What are the stages of skin aging?

It is more useful to think in phases than in ages: first the surface and fine lines change, then firmness, then structure. When each phase arrives depends on genetics and, above all, on sun.

Do pores get bigger with age?

They tend to become more visible, mainly on the cheeks, because the surrounding skin loses firmness and the pore elongates. In young skin, the cause is usually oil.

Can you lower your skin age?

You can improve the signs that make up that age: fade spots, improve texture, stimulate collagen. Clock-driven aging continues. What you control is how much sun and habits add to it.

What is the best treatment to rejuvenate facial skin?
