Ultherapy vs Ultraformer: how to choose between them
Both are microfocused ultrasound and do the same thing on paper. The difference lies in how the energy is delivered, in comfort, and in who operates the device. How to decide.
The question almost always arrives in the same form: which one is better. The honest answer is that it's the wrong question.
Both are microfocused ultrasound. Both deliver thermal energy to deep layers of the skin to trigger a repair response: the body reads that controlled heat as a stimulus and produces new collagen there. The mechanism is the same. What changes is how each platform delivers that energy, how much control the practitioner has over depth, and what that costs in comfort during the session.
I'll go in the order that matters for a real decision, not the order the manufacturers would prefer.
What the two have in common
Before comparing, it's worth pinning down what doesn't change from one to the other:
- Both are microfocused ultrasound. Not laser, not radiofrequency, not light. It is mechanical energy converted into heat at specific points, sparing the surface of the skin.
- Both work by stimulating collagen. Neither one "tightens" the skin on the spot. What you see in the mirror weeks later is new tissue, produced by your own body.
- Both are in-office procedures, with no incision and no downtime. You come in, have it done and leave. There may be redness for a few hours and local tenderness for a day or two.
- Neither is a facelift. This bears repeating, because it is the source of most of the disappointment. Microfocused ultrasound improves firmness and contour definition. It does not remove excess skin.
If your case calls for a facelift, the choice between the two machines is irrelevant. Neither will deliver it.
What the difference between the devices is
The real difference comes down to three points.

The first point is the one almost nobody explains properly. Ultherapy shows the layer being treated on screen before the energy is released. That matters because tissue thickness varies a great deal from face to face, and the same depth that hits the right layer in one person can hit fat, or nothing, in another.
The second point is what gets Ultraformer sold as more versatile: more depth options, including very shallow layers, make it possible to work on texture and thin areas.
The third point, pain, follows from the first two. Energy delivered deeper hurts more. A more comfortable session may mean the platform is gentler, or it may mean the energy isn't reaching where it needs to go. You can't read comfort as quality, or as a flaw. You can read it as information.
What treatment is better than Ultraformer
Put that way, the question assumes there is a ranking. There isn't. There is suitability.
What determines the result, in order:
1. The indication being right. Microfocused ultrasound in someone who needs surgery delivers disappointment on any device.
2. Who operates it. This is the most decisive factor and the least discussed. Marking, choice of depth, density of shots, reading of the face. A good operator on a middling platform delivers more than a poor operator on the best machine on the market.
3. The protocol applied. Delivering less energy than needed so it "doesn't hurt" is the most common cause of a weak result. It shows up in sessions that are cheap, quick and comfortable.
4. The platform. Yes, it is the fourth item. Not the first.
Choosing a clinic by the name of the device is choosing by the least important item on the list.
Which one we use at RUV, and why
At RUV we use Ultherapy, by Merz. The choice comes down to predictability: seeing the layer before firing reduces the chance of delivering energy in the wrong place, and in a procedure whose result only appears weeks later, hitting the wrong layer is a mistake you only discover late.
We use every transducer, and it is the transducer that defines the action, not the machine:
- the more superficial transducers stimulate collagen;
- the deeper ones act on fat.
That's why the same device shows up in a plan for sagging and in a plan for a double chin. What changes is the depth at which the energy is delivered. How long Ulthera results last
We don't work with a fixed timeline, and you should be wary of anyone who does. The result depends on how much collagen your body produces, and that varies with age, genetics, smoking, sun exposure and skin quality.
What can honestly be said:
- The change is not immediate. New collagen takes weeks to organize. Judging the result the following week means judging swelling.
- The gain is progressive over the following months, up to a plateau. From there on, what you see is the result.
- Aging continues. The procedure doesn't freeze anything. It restores firmness on top of a process that keeps going, which is why maintenance exists.
Anyone promising an exact number of months is selling a predictability that biology doesn't offer.
Can you get Ultraformer while pregnant
No. And that applies to any microfocused ultrasound, including Ultherapy.
It's not because there is proof of harm. It's because there is no study that allows anyone to claim safety during pregnancy, and an aesthetic procedure is elective. When there is doubt about risk in a procedure that can simply wait, the decision is to wait. The same logic applies to breastfeeding.
This is the answer a clinic that wants a full schedule doesn't like to give. It is the right answer.
Ulthera or Ultherapy: is it the same thing?
It is. The device is the same: Ulthera is the name the machine became known by in Brazil, and Ultherapy is the name of the treatment. Anyone asking "Ulthera or Ultherapy" is comparing a name with itself.
The confusion is worth knowing about for a practical reason: "Ulthera" is sometimes used to describe sessions done on other devices, because the name caught on as a synonym for microfocused ultrasound. Ask which platform is being used, not what the procedure is called.
What goes into the cost
We don't publish prices, but we can explain what makes up the bill, and why comparing prices between clinics without that information tells you nothing.
The cost of a microfocused ultrasound session is driven by two factors: the number of shots and which transducers were used. Transducers are consumables, with a cost per shot. That's what explains the huge gap between quotes for "the same thing".
A session with half the shots costs half, delivers less and leaves the clinic under the same name. Before comparing prices, ask how many shots and in which areas. Without that, you're comparing two different things.
Safety
Microfocused ultrasound doesn't enter a vessel and doesn't inject anything, so its risk profile is different from filler. Even so, it is energy delivered at depth into a face that has vascular pathways, nerves and, often, filler material from earlier treatments.
That's why a Doppler ultrasound assessment is part of the process here. Before treatment, it shows the structure of that particular face, including old filler, which patients don't always remember to mention and which changes the plan. After treatment, when the plan combines both approaches, it confirms there is no vessel compression. That is what turns "it's safe" from a promise into a check.

When we advise against it
- When the case is surgical. Significant excess skin doesn't respond to collagen stimulation. We refer you elsewhere, and we say so at the assessment, not after three sessions that changed nothing.
- Pregnancy and breastfeeding. An elective procedure waits.
- When the expectation is an immediate result. Collagen stimulation doesn't work on that timeline. Aligning expectations beforehand avoids disappointment afterward.
- When there is infection, an active lesion or a wound in the area. We wait for the skin to recover.
- When someone wants "just one session to try it" before an upcoming event. The timeline doesn't fit, and the money gets spent before there's time to see what it bought.
- When the complaint is texture, spots or pores. That calls for a different approach: peels, laser, skin boosters. Microfocused ultrasound works on firmness and support.

Which is better, Ultraformer or Ultherapy?
It depends on the case and, above all, on who performs it. Ultherapy has real-time visualization of the layer, which gives more predictability in people whose anatomy is less obvious. Ultraformer has more depth options and is usually more comfortable during the session. Neither makes up for the wrong indication or a cost-cutting protocol.
Does Ultraformer really work?
It does, within what microfocused ultrasound does: firmness, contour definition, collagen stimulation. It doesn't work as a facelift. Most "it didn't work" stories come down to one of three things: a case that was really surgical, less energy delivered than needed, or a result judged too early.
What is the strongest treatment for sagging skin?
Without surgery, microfocused ultrasound is the most consistent option for deep support. But "strongest" isn't a useful criterion. The useful criterion is which approach addresses your particular kind of sagging. Loose skin and loss of contour call for different things, and sometimes combining technology with projection delivers more than any device on its own.
Liftera or Ulthera, which should I choose?

Does one hurt more than the other?
Ultherapy is usually more uncomfortable, especially in the deep layers. The discomfort happens during the shot and passes quickly. It's worth knowing that a session with no discomfort at all in the deep layer raises the question of how much energy was actually delivered.
Can I have it if I already have filler in my face?
In most cases yes, and that's exactly why the Doppler assessment exists: knowing where the material is before delivering energy changes the plan. What you can't do is treat without knowing.
How many sessions will I need?
It depends on the degree of sagging and on individual response. Many cases are resolved with one well-done session followed by maintenance. A clinic that sells a fixed package before looking at your face is selling appointments, not a plan.
