Microfocused ultrasound: what it does and what it doesn't
Microfocused ultrasound delivers energy deep beneath the skin to stimulate collagen and act on fat. What it fixes, how long it takes, and when it isn't worth it.
Microfocused ultrasound is probably the most poorly explained aesthetic procedure there is. Half of what's written about it sells it as a lift without surgery. The other half dismisses it as doing nothing. Both readings get it wrong for the same reason: they treat the device as if it had a single function.
It has two. And the difference between them lies in the depth at which the energy is delivered, not in the machine.
What microfocused ultrasound does
It does two things that tend to show up together in the same face:
- Stimulates collagen, when the complaint is laxity, thin skin, or a contour that has lost firmness.
- Acts on fat, when there is volume in an area where it blurs the contour — a double chin, a heavy lower third.

How microfocused ultrasound works
Microfocused ultrasound concentrates energy at specific points beneath the skin without breaking the surface. At those points the tissue receives a controlled stimulus, and the body responds by producing new collagen to reorganize the area.
Two practical consequences of this mechanism:
The skin isn't wounded. There is no cut, no peeling, no mandatory recovery period. The energy passes through the surface and concentrates below it.
The result comes from your body, not the device. The device delivers the stimulus; you are the one producing collagen. Many people search for "Ultraformer full face" thinking it's a different procedure. It isn't. Ultraformer is the name of a microfocused ultrasound device — just as Ultherapy is the name of another. "Full face" only describes the extent of the treatment: the whole face, rather than a single area.
The difference that actually matters: what separates a good treatment from a bad one isn't the brand on the device, it's the treatment plan — which layers, at which points, at what density of pulses. Two practitioners with the same device deliver different results because of this.
And "the whole face" deserves a caveat. Treating everything isn't always better than treating where the face needs it. Here the assessment always covers the whole face — but the treatment follows what the structure calls for, not a fixed protocol sold by the session.
How long microfocused ultrasound results last
The honest answer has two parts, and both tend to get distorted.
It isn't permanent. The new collagen is real, but aging keeps happening underneath. What the procedure does is improve the current condition and slow the curve — not freeze the face at one point.
It isn't short-lived. The collagen that forms organizes itself and holds for a long period, in the range of extended months, and maintenance is usually yearly or close to it, depending on the face and the age.
I won't give a fixed number here, because a fixed number on this topic is a guess dressed up as precision. The real variation between a 35-year-old with thick skin and a 60-year-old with accumulated sun damage is too large to fit into an average.
How long until results show
This is the question that causes the most frustration, because expectations come from filler — where the result shows immediately.
Here it doesn't. Collagen takes weeks to organize, and an honest reading of the result happens after that time, not the following week. A specialist from the Quirónsalud group describes the optimal result at around 6 to 8 weeks after treatment, when skin tightening and redefinition of the facial contour are achieved.
In practice, this is how it behaves: in the first weeks almost nothing changes visually. After that, the skin starts to read differently — firmer, with a more defined contour. Anyone who takes a photo the following week and concludes it didn't work is assessing too early.
How many sessions are needed
It depends on what is being treated and on the starting point. Mild laxity in skin with good responsiveness may be resolved in one well-planned treatment. More established laxity usually calls for more than one, spaced out to give collagen time to form in between.
What makes no sense is buying a fixed package before seeing how your face responds to the first treatment. Your individual response is the information that defines the plan — and it only exists afterward.
What drives the cost
We don't quote prices here, but we can explain what weighs on them. Three things: the number of pulses — which depends on the area treated and the density the case calls for —, the transducers used, since each layer requires its own, and the time spent on assessment and planning before the treatment.
That is why a microfocused ultrasound quote over the phone means nothing. Without knowing the area and the density, the number is fiction.
Botox or microfocused ultrasound
A common question, and the comparison doesn't hold up because the two solve different problems.

A forehead line that deepens with movement won't be solved with ultrasound. Loose skin in the lower third won't be solved with toxin. And in a large number of faces, both belong to the same plan, at different moments.
What are the downsides
The part promotional material leaves out:
- Results take time. If you want a visible change for an event in two weeks, this is the wrong procedure.
- Response varies. Heavily damaged skin or advanced laxity respond less, and sometimes the gain doesn't justify the procedure.
- There is discomfort during treatment. Deep energy is felt. It's tolerable and ends when the treatment does, but it isn't a quiet procedure.
- It doesn't replace surgery. There is a point of laxity at which no technology delivers what the person wants, and insisting is spending time to reach a lukewarm result.
On that last one: when the case is surgical, we refer to a partner surgeon. We say so at the assessment, not after three sessions that changed nothing. Sometimes the best procedure is the one we don't do.
Safety
Microfocused ultrasound injects nothing and doesn't break the skin, which puts it in a lower risk category than filler. Even so, planning goes through the same structural assessment we do for any procedure.
Assessment by Doppler ultrasound is part of the process here: before, to understand the structure of that face — including filler material that may already be there from previous treatments, which changes the plan — and after, to confirm there is no vessel compression. In someone who has had filler and is about to have deep-energy technology, knowing where the material sits is information that changes the treatment. That is what turns "it's safe" from a promise into a verification.

- When the expectation is an immediate result. Collagen doesn't work on that timeline, and aligning on this beforehand prevents disappointment later.
- When laxity is advanced. Technology has a limit. Forcing it delivers frustration and cost with no return.

- Pregnancy and breastfeeding. An elective procedure waits. It's not alarm, it's criteria.
